Cancellation Policy: If you need to cancel or re-schedule your appointment please observe our 24 hour cancellation policy.

 

Arriving early for your appointment: 
If you are new to our office, please allow an extra 15 minutes prior to your appointment to fill out necessary paperwork.  Please come prepared with a copy of your insurance information. To speed up the process you can download and fill out our new patient forms listed here.

If you are a new patient to Beach Cities Orthopedics and Sports Medicine, please fill out a New Patient Intake Form (regular or minor) as well as a New Patient Medical History Form.  If you have a specific injury to the spine, lower body or upper body; please fill out the corresponding form as well and bring it to your first appointment.
